The Transition Management Assistant/ UM Administration Coordinator 4 contributes to administration of Transition of Care in both utilization management and care management. The UM Administrative Coordinator 4 assumes ownership and leads advanced and highly specialized administrative activities including, various enrollment and disenrollment compliance reporting, evaluating and reporting transition of care trends, and facilitating transitions between Humana and Care Coordination entities.

The Transition Management Assistant /UM Administration Coordinator 4 provides clinical and non-clinical support for the policies and procedures ensuring best and most appropriate treatment, care or services for the members.
Provide transition support by managing shared mailboxes, troubleshooting and escalating member/provider concerns, and entering documentation into the care management documentation system.
Responsible for analyzing and compiling various utilization and care management reports for submission to Compliance in accordance with the Provider Agreement.
Develop and maintain relationships with external care coordination entities, functioning as a point of contact regarding member transitions.
Access multiple internal and external systems and portals
Complete other administrative functions, as assigned
3+ years administrative or care coordination experience within the healthcare industry
1 + years demonstrated capability with mentoring, coaching and/or developing associates formally or informally
Advanced knowledge of MS Office including Word, Excel and ability to quickly learn new systems.
Proficient utilizing electronic medical record and documentation systems
Proficient and/or experience with medical terminology
Excellent verbal and written communication skills
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Experience as a mentor, trainer and/or preceptor
Experience in a managed care organization
Prior member service or customer services telephone experience
Workstyle: Remote work at home
Location: Must reside in Ohio
Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM Eastern Time, Over-time. Hours subject to change based on business needs
Travel: 5+% occasional travel to office for staff meetings, training, etc.

If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone or computer to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters. Associates who remain unvaccinated must either undergo weekly negative COVID testing OR wear a mask at all times while in a Humana facility or while working in the field.
WAH requirements: Must have the ability to provide a high speed DSL or cable modem for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.
A minimum standard speed for optimal performance of 25×10 (25mpbs download x 10mpbs upload) is required.
Satellite and Wireless Internet service is NOT allowed for this role.
