Job Description :
At PrimaryOne Health, it is our mission in Central Ohio and surrounding communities to provide access to services that improve the health status of families—including people experiencing financial, social or cultural barriers to healthcare. PrimaryOne Health is currently seeking a Community Health Outreach Worker acts as a liaison between PrimaryOne Health, community programs and services and community members. The focus for the Community Health Outreach Worker is on health, education, promotion, assistance and referral. Provides outreach services to individuals within identified communities and vulnerable populations; help obtain a variety of social and personal services for clients. Determines eligibility for public assistance programs, Affordable Care (as a certified application counselor), and Medicaid; links persons to PrimaryOne Health services and provides information/referral to other social and human services for patients and residents. Education: High school diploma or equivalent; some college preferred. Community Health Worker certification preferred. Experience: One (1) year of relevant community service experience preferred.

Member Duties : The VISTA Member would be primarily responsible for engaging CMHA residents within the RISE center. The resource center located within a CMHA location will allow the VISTA Member to speak to many residents, provide expedited access to primary care and assist in addressing varying needs of the residents through referrals to agencies providing direct services to meet those needs. The VISTA Member would also be instrumental in linking residents to transportation services to and from medical appointments. Located within a central location, the VISTA Member would also provide information regarding the support services available through PrimaryOne Health. For example, this would include Diet/Nutrition services to patients with diabetes and hypertension; Pregnancy Centering services to support expectant mothers; Fresh food and produce through our “farmacy” food pantry program. These types of programs and services provide tangible benefits over and above direct health care.
